A BC nurse is scheduled for a hearing next month after he was accused of giving the wrong fentanyl dose to two patients.
The nursing regulator said incorrect doses of fentanyl patches were given to each patient on the same day, July 5, 2022 and nurse Mohammed Ghasemi Chaleshtari could face consequences for professional misconduct.
Both patients were using the patches for opioid antagonist therapy, but it's not clear whether Chaleshtari gave them more or less than their prescribed dose.
Chaleshtari, a registered practical nurse at a Surrey opioid clinic, was given notice of his September hearings on June 12, but the BC College of Nurses and Midwives published the hearing notice on Aug. 21.

He was previously suspended for two weeks on June 12, 2022, when he falsified a patient's records, according to a College consent agreement.
On Dec. 5, 2021, he concealed a medication error after giving a patient too much morphine. He also failed to tell the patient or monitor for the increased risk of overdose.

He signed the consent agreement, as often occurs with the College, and was given the suspension along with other conditions that included a professional ethics course.
This time, Chaleshtari won't be signing a consent agreement and the College will host a five-day hearing where he can answer for the accusations.
He doesn't have any current restrictions on his license and is allowed to practice.
